This form is so that you give permission for another non-parent
person/relative to bring your child into the
office to get medical care. We cannot see them unless you give that
person permission.

your previous doctor, then fill out this

You will need to fill these out at your first office visit. You can
print these out at home and fill them out ahead of time. Then bring
them in with you to the office.
